Don't you like to know a little about your clients before you meet them?  I'm the same way.  Also, safety is important to me.  In other words, I simply need to know that you are who you say you are.  Appointments cannot be arranged without completing this form.  If you do not want to fill it out on-line, you can provide me the same information when we speak.  It's called verification, which can usually be accomplished through employment verification.  Higher caliber ladies expect to verify their clients.

If you're not willing to provide this information, we cannot be meet.  Please be prepared to show government issued ID at our first appointment.
